Hospice Nurse Case Manager

We are seeking a dedicated and compassionate Hospice Nurse Case Manager to join our esteemed healthcare team. The Hospice Nurse Case Manager plays a pivotal role in delivering high-quality palliative care to patients and their families in a home-based or facility setting. This position requires a deep understanding of end-of-life care and the ability to provide emotional support and guidance to both patients and their loved ones during a challenging time. The selected candidate will be responsible for comprehensive assessments, care planning, and coordination of services to ensure an optimal quality of life for patients facing terminal illness. The Hospice Nurse Case Manager must demonstrate exemplary clinical skills, strong communication abilities, and a commitment to patient-centered care. Additionally, the individual will collaborate closely with interdisciplinary teams to align care objectives with the needs and preferences of patients. Providing education and resources to families and caregivers is also a critical aspect of this role. As a representative of our hospice program, the Hospice Nurse Case Manager must uphold our values of empathy, respect, and integrity, contributing to a supportive environment for patients and families alike.

 

Hospice Nurse Responsibilities:

  • Overseeing the care of patients who are nearing the end of their lives.
  • Including, but not limited to; scheduling patient visits in accordance with needs, physician collaboration, orders management, medication management
  • Supervises LPN’s / LVN’s / Hospice Aides per protocol
  • Work with a team of medical professionals to ensure that these individuals receive high-quality palliative care, including physical, emotional, and spiritual support.

Qualifications:

  • Associates degree, preferred
  • Registered Nurse license required
  • Minimum one year of experience 
  • Previous experience in hospice
  • Willingness to serve on-call
  • Must have reliable transportation & valid driver’s license
