You will be responsible for monitoring patient progress, coordinating with other healthcare professionals, and adapting care plans as necessary. Responsibilities

  • Conduct comprehensive patient assessments to determine healthcare needs.

  • Administer medications and treatment as prescribed by physicians.
  • Monitor and document patient progress and response to treatment.
  • Educate patients and caregivers on disease management and health maintenance.
  • Coordinate with interdisciplinary teams to ensure comprehensive patient care.
  • Respond to patient inquiries and provide support in a timely manner.

Qualifications

  • Active (unrestricted) nursing license in the State of California
  • 1 year (professional) nursing experience in Home Health or Acute Care
  • Valid Driver’s License w/ Clean DMV
